What it is
Gluten proteins in wheat, barley, and rye trigger an immune response that injures the small-intestinal lining in susceptible people. Damage can reduce absorption of nutrients and produce effects throughout the body. Celiac disease is autoimmune, not the same as an immediate wheat allergy, and not diagnosed solely by feeling better off gluten. Because treatment is demanding and affects testing, nutrition, family screening, and daily life, establishing the diagnosis accurately before long-term restriction is important.

Symptoms and how it may present
Symptoms differ widely. Diarrhea, constipation, abdominal pain, bloating, gas, nausea, vomiting, weight loss, or greasy stools may occur, but some people mainly have iron-deficiency anemia, fatigue, mouth ulcers, low bone density, infertility, headaches, numbness, an itchy blistering rash called dermatitis herpetiformis, poor growth, or delayed puberty. Others have few obvious symptoms despite intestinal injury. Symptoms after wheat are not automatically celiac disease: wheat allergy, irritable bowel syndrome, inflammatory bowel disease, infection, lactose intolerance, and non-celiac wheat or gluten sensitivity require different evaluation.

Causes and risk factors
Celiac disease requires genetic susceptibility, usually involving HLA-DQ2 or DQ8, plus gluten exposure and immune activation. These genes are necessary in almost all cases but are common in people who never develop disease, so they predict possibility rather than destiny. Risk is higher in first-degree relatives and in people with type 1 diabetes, autoimmune thyroid disease, selective IgA deficiency, Down syndrome, Turner syndrome, or certain other autoimmune and genetic conditions. Neither ordinary bread consumption nor parenting causes the disease.

Diagnosis and tests
Tests should answer a clinical question and be interpreted together with symptoms, examination, and history.
Testing should normally happen while the person is still eating gluten; starting a gluten-free diet first can make blood tests and biopsy appear falsely reassuring. Initial tests often include tissue-transglutaminase IgA and total IgA, with alternative antibody tests when IgA is low or the situation requires them. Most adults with positive or strongly suspected disease undergo upper endoscopy with several small-intestinal biopsies. Genetics for HLA-DQ2 or DQ8 cannot prove celiac disease because these types are common, but their absence makes it very unlikely. Children may follow specialist criteria that sometimes avoid biopsy in carefully defined cases.

Treatment options
The best option depends on severity, other conditions, likely benefit and harm, access, and personal priorities.
The treatment is complete avoidance of gluten from wheat, barley, and rye. A celiac-informed dietitian can teach label reading, balanced substitutions, cross-contact prevention, eating outside the home, travel, school or workplace planning, and how to avoid unnecessary restriction. Naturally gluten-free foods such as rice, corn, potatoes, beans, vegetables, fruit, eggs, unprocessed meat, fish, nuts, and suitable dairy can form the base according to individual needs. Oats should be specifically labeled gluten-free and may not suit everyone. Medicines rarely replace the diet, although complications or persistent disease may need specialist treatment.

Daily life and supportive care
At diagnosis, care may assess iron, folate, vitamin B12, vitamin D, calcium, liver tests, blood count, weight and growth, bone health, vaccination needs, and other autoimmune conditions. Deficiencies should be treated to a measured need rather than with an indiscriminate supplement stack. Check every vitamin, probiotic, herbal product, medicine, and food supplement for gluten and safety. Persistent symptoms deserve a systematic review of diagnosis, accidental exposure, lactose or other food intolerance, microscopic colitis, pancreatic problems, infection, irritable bowel syndrome, and rare refractory celiac disease instead of escalating restriction without evidence.

Monitoring and follow-up
Follow trends that can change a decision rather than collecting measurements without a purpose.
Follow-up reviews symptoms, growth or weight, diet quality and cross-contact, adherence barriers, and whether antibody levels are falling. Blood counts, iron, folate, B12, vitamin D, liver tests, thyroid measures, and bone health are checked according to the baseline problem and recovery. Repeat biopsy is not automatic for everyone but may be considered when symptoms, antibodies, or healing are concerning. Dietitian review matters because a diet can be technically gluten-free yet low in fiber, iron, folate, or other nutrients and unnecessarily expensive.

Complications and long-term outlook
With a reliable gluten-free diet, antibodies usually fall, the intestine heals, symptoms improve, and long-term complication risk declines. Healing may be slower in adults, and symptom recovery does not always match biopsy recovery. Small accidental exposures can cause symptoms or renewed inflammation but should be addressed with investigation and practical prevention, not guilt. Family members may need testing before changing their diet. Rare refractory disease, persistent malnutrition, or concern for intestinal cancer requires specialist gastroenterology evaluation.

When to seek urgent help
Most people improve as the intestine heals, but recovery and antibody decline can take time. Untreated disease raises risks of malnutrition, anemia, osteoporosis and fractures, reproductive problems, neurological complications, and rare intestinal cancers or lymphoma. Seek urgent care for severe dehydration, repeated vomiting with inability to keep fluids down, vomiting blood, black or bloody stools, fainting, severe weakness, a rigid or intensely painful abdomen, or new major neurological symptoms. A small accidental exposure usually does not require emergency care, but severe allergy-like symptoms may indicate a different condition and need immediate assessment.

Questions to discuss with a care team
A guideline describes what tends to help groups of people, but it cannot decide what matters most in one person’s life. Revisit the plan when symptoms, function, other illnesses, pregnancy plans, medicine access, side effects, or personal priorities change. Ask for plain-language reasoning and say directly when cost, time, transport, caregiving, or treatment complexity makes a recommendation unrealistic.
