What it is
Rheumatoid arthritis occurs when persistent immune inflammation targets the synovial lining of joints. Over time the inflammation can damage cartilage, bone, tendons, and surrounding structures, while systemic inflammation can affect health beyond joints. It is distinct from osteoarthritis, although both can coexist. Modern care aims for remission or low disease activity, preserved participation in daily life, and prevention of treatment harm—not merely making pain temporarily tolerable.

Symptoms and how it may present
Rheumatoid arthritis often causes pain, swelling, warmth, and prolonged morning stiffness in several joints, commonly the hands, wrists, or feet and often on both sides. Fatigue, low-grade fever, reduced appetite, and loss of strength can accompany a flare. Symptoms can come and go, and the pattern may differ early in the disease. Osteoarthritis, gout, viral arthritis, psoriatic arthritis, lupus, infection, and other conditions can resemble it. Persistent swollen joints matter more diagnostically than pain alone, and a normal-looking day does not erase a documented inflammatory pattern.

Causes and risk factors
Genes, immune regulation, hormones, and exposures interact; there is no single preventable cause. Tobacco exposure is a well-established modifiable risk and is associated with some antibody-positive disease, while gum disease, silica exposure, body composition, and other factors are under study or relevant in particular groups. Women are diagnosed more often, but anyone can develop it. Family history changes probability but does not make the disease inevitable. Diet, stress, cold weather, or an injury may influence symptoms but should not be blamed as the cause.

Diagnosis and tests
Tests should answer a clinical question and be interpreted together with symptoms, examination, and history.
Diagnosis is clinical and has no single yes-or-no test. A clinician examines which joints are swollen or tender, how long stiffness lasts, how symptoms developed, and whether other organs are involved. Blood tests may include rheumatoid factor, anti-CCP antibodies, inflammatory markers, blood counts, kidney and liver function, but positive antibodies can occur without rheumatoid arthritis and some affected people remain antibody-negative. Ultrasound, X-ray, or MRI may identify inflammation or damage. Joint fluid is sometimes tested urgently to exclude infection or crystals, especially when one joint becomes suddenly hot and swollen.

Treatment options
The best option depends on severity, other conditions, likely benefit and harm, access, and personal priorities.
Disease-modifying antirheumatic drugs, called DMARDs, treat the inflammatory process rather than only masking pain. Methotrexate is a common first medicine for moderate or high disease activity, with folic acid and safety monitoring; hydroxychloroquine, sulfasalazine, leflunomide, biologic medicines, and targeted synthetic DMARDs have roles depending on response and risk. Short glucocorticoid courses may bridge to slower treatment, but long-term exposure has important harms. Pain medicines can help symptoms but do not prevent joint injury. The plan uses treat-to-target reassessment and shared decisions about benefit, infection risk, pregnancy, cost, and burden.

Daily life and supportive care
Movement is usually part of treatment, not a cause of joint destruction. A graded mix of aerobic activity, strength, mobility, hand exercises, and rehabilitation can protect fitness and function; during a severe flare the program may need temporary modification rather than complete inactivity. Occupational therapy, joint protection, assistive devices, sleep support, vaccines, dental care, stopping tobacco, and cardiovascular risk management are useful. No supplement or restrictive diet replaces a DMARD. Discuss fish oil, herbs, high-dose vitamins, and anti-inflammatory products because evidence, purity, bleeding, liver effects, and interactions vary.

Monitoring and follow-up
Follow trends that can change a decision rather than collecting measurements without a purpose.
At planned visits, the team measures swollen and tender joints, symptoms, function, and sometimes inflammatory markers to judge whether the agreed target has been reached. Medicine-specific monitoring may include blood counts, liver and kidney tests, infection screening, eye examinations, skin checks, or lipid testing. Vaccination, pregnancy and contraception plans, bone protection, dental care, lung symptoms, cardiovascular risk, tobacco exposure, and mental health also belong in follow-up. A test should have a purpose and a clear response rather than becoming routine collection without action.

Complications and long-term outlook
Outcomes have improved substantially with early DMARD use and adjustment to a target, but response is individual and some damage may remain. Long periods of low activity or remission are possible; medicines may sometimes be reduced cautiously, but abrupt stopping can trigger relapse and should be a shared decision. Function is shaped by inflammation, structural damage, fitness, pain processing, sleep, mood, work demands, and access to therapy. Good care therefore treats the whole burden while continuing to suppress damaging inflammation.

When to seek urgent help
Uncontrolled inflammation can erode joints and contribute to disability, osteoporosis, cardiovascular disease, lung disease, anemia, eye inflammation, nodules, or nerve compression. Medicines can also lower infection defenses or affect blood counts, liver, kidney, skin, or clotting, so risks come from both disease and treatment. A hot swollen joint with fever, severe breathlessness, chest pain, coughing blood, sudden weakness, painful red eye with visual change, or signs of serious infection during immune treatment needs urgent assessment. Do not assume every fever or painful joint is simply a flare.

Questions to discuss with a care team
A guideline describes what tends to help groups of people, but it cannot decide what matters most in one person’s life. Revisit the plan when symptoms, function, other illnesses, pregnancy plans, medicine access, side effects, or personal priorities change. Ask for plain-language reasoning and say directly when cost, time, transport, caregiving, or treatment complexity makes a recommendation unrealistic.

- What is the treatment goal, and how do the main options differ in benefit, harm, burden, and cost?
